The questions file should be edited in a plain text editor such as Notepad or WordPad. Unfortunately, Word messes things up. For some FAQs on the questions.xml file, see this thread.

When I open the questions.xml document in Microsoft Word, I see question and answer tags. I right-click on the question tag and click on attributes to edit the question. However, I can only delete the original question and cannot add a new one. When I try to modify it, my changes do not save.

What is the right process for editing the questions.xml document in Microsoft Word?
