While I recommend the painless manual insert method, most people use the automatic JavaScript positioning. As outlined in this post by Midwestern City Boy, the positioning of the anti-spam fields were output below the comments label for themes that have such a label. This release makes it so that the anti-spam fields are always above the comments label.

If you are upgrading, simply extract the new files on top of the old files. For this plugin, it is not recommended to use the automatic WordPress plugin upgrade feature.
