Thank you so much. How do I embed on one of my pages. When I copy and paste the php script into a test.html I can see the code on the web when I send over via ftp.

You could change the look of the WordPress login page, but that's a CSS issue and/or something that other plugins probably provide.

You could also 1) embed the login form on another page or template and 2) have a redirect log out, explained somewhat here.

I searched for several days for exactly what you provided...awesome. One question, can I change the look of the page to resemble my home page, which is:
